Key features of our Aftercare Service include

Barróg Healthcare Aftercare Service is not just about support—it’s about empowerment, growth, and preparing individuals for long-term success in the community. Our model is designed to nurture independence at every step.

We provide high-quality, furnished single-occupancy units, promoting autonomy and creating a space where individuals can feel secure and at home, while still having access to care and support when needed.

Each individual receives a customised support plan, designed to address their unique needs and goals. This ensures the right balance of support and independence in a community-based setting, encouraging active participation in local life.

Our experienced and compassionate staff are available 24/7, with varying levels of intervention depending on assessed needs. From low-level check-ins to high-level, hands-on assistance, we adapt to the individual’s progress and preferences.

Access to the service is based on a robust assessment process, which evaluates readiness for semi-independent living, risk factors, and support requirements. We collaborate closely with social workers, mental health teams, and the individual to determine suitability and develop a transition plan.

As service users grow in confidence and capability, we offer a graduated reduction in support, empowering them to manage more aspects of daily life independently. This dynamic approach ensures care remains relevant without fostering dependency.

We place a strong focus on building life skills—such as budgeting, cooking, cleaning, and time management—and fostering self-management capabilities, equipping individuals to lead fulfilling, independent lives.

At Barróg Healthcare, our Shared Accommodation services provide a supportive, community-based living environment designed to foster personal growth, independence, and social connection. These homes offer shared facilities while promoting individual development and well-being.

Our shared accommodation models are designed to promote a sense of belonging and mutual respect. Residents live in safe, homely environments where shared spaces encourage interaction, inclusion, and a community ethos.

Living alongside peers provides natural opportunities for emotional support, shared experiences, and social learning. We actively promote social integration through structured group activities and informal communal living.

We maintain appropriate staff-to-resident ratios based on individual needs, risk assessments, and placement goals. Our skilled support teams offer guidance and supervision without compromising residents’ autonomy.

Each resident follows a personalised support and placement plan focused on developing core life skills—such as budgeting, cooking, self-care, and decision-making—to prepare them for more independent living in the future.

Barróg Healthcare is committed to empowering individuals to progress through our service pathway. Our accommodation is structured to act as a stepping stone toward greater independence or fully independent living, in line with each person’s aspirations and capabilities.
