Outreach Services for Persons with Disabilities

At Barróg Healthcare, our Disability Outreach Services are built on the belief that every individual—regardless of age or ability—deserves the opportunity to lead a fulfilling, independent, and inclusive life.

We provide tailored, person-centred support to children, young people, and adults with a range of physical, learning, and developmental disabilities. Our outreach team works within the community, delivering support that extends beyond traditional care settings and into everyday environments where people live, learn, work, and socialise.

Whether assisting with daily living skills, promoting independence, accessing education or employment, or supporting involvement in social and recreational activities, our focus is always on empowering individuals to achieve their goals, participate fully in their communities, and enjoy meaningful connections.

With a compassionate, flexible, and inclusive approach, Barróg Healthcare Disability Outreach Services help bridge the gap between support needs and community participation—ensuring the people we support are not only cared for, but truly seen, heard, and valued.

Person-Centred Planning at Barróg Healthcare Disability Outreach Services

At Barróg Healthcare, our Disability Outreach Services are guided by a Person-Centred Planning approach. We are committed to ensuring that every individual we support is at the heart of their own care, with a focus on dignity, independence, and personal growth.

We begin by carrying out a thorough and personalised assessment to understand the unique goals, aspirations, and support requirements of each individual. Working in partnership with the person and their support network, we develop meaningful, achievable goals that reflect their values and lifestyle.

Barróg Healthcare empowers individuals to have real choice and control over the decisions that affect their lives. From selecting support staff to planning daily routines, we encourage active participation in all aspects of decision-making to promote independence and self-determination.

Our team uses a strengths-based approach to focus on what individuals can do, not just what support they need. We identify and build upon existing abilities, talents, and interests to foster confidence, skill development, and community engagement.

Support plans at Barróg Healthcare are regularly reviewed in collaboration with the individual, their family or carers, and key professionals. We ensure plans remain flexible and responsive to changing needs, circumstances, or goals—keeping the person’s voice at the centre of every update.
